The COVID-19 pandemic is an unprecedented challenge we are facing locally and globally. Now, more than ever, we must collaborate with our community members and community-based organizations in innovative ways in order to protect our most vulnerable populations. We are pleased to announce the launch of a coordinated care network - Unite Virginia! Unite Virginia is sponsored by Virginia Department of Health and Human Resources in partnership with the Virginia Department of Health and the Virginia Department of Social Services, Optima Health, Virginia Hospital and Healthcare Association, Partnering for a Healthy Virginia, and Kaiser Permanente.
Unite Virginia will connect community-based organizations, health care providers, and public agencies through Unite Us’ shared technology platform to address the social needs that have an impact on individual and community health and well-being. The platform is designed to make your current referral workflows more efficient, trackable and secure. You will have enhanced visibility outside of your four walls and the ability to ensure the individuals you serve are connected to the services they need.
The Unite Virginia network will contain partners who provide a broad range of services such as housing, employment, food assistance, behavioral health, utilities, and more. As a partner, you will have access to both organization-level data as well as aggregate, community-wide level data to better understand the needs of community members and gaps in service availability to address those needs.
Participation in this coordinated care network is free for community-based organizations.
